Opening

This is the certify-then-reject loop. The searches happening right now do not use that name. They sound like this: the pipeline is green and production is broken. The agent said it fixed the work, and the work is still wrong. Code review keeps passing an artifact a named human then refuses.

The name

A certify-then-reject loop is a repeating failure pattern in which an automated gate records a passing result on an artifact, and a human with accountability for that artifact then rejects it on inspection. A repeat count above one is diagnostic: it establishes that the certification method is measuring something other than the property the human is judging. The loop is detectable without reading application code, because both events — the automated pass and the human rejection — are already recorded in ordinary version control, pipeline and issue-tracker history.

A single reject after a pass can be taste, timing, or a missed requirement. A repeat count above one is the diagnostic. The gate and the human are scoring different properties, and the organization is paying for both scores as if they agreed.

Why it repeats

Automated certification is fast, consistent, and blind to the property the accountable human is actually judging. The human rejection is slow, expensive, and the only score the customer, the regulator, or the next downstream owner will accept.

Each reopen writes another passing certification onto the same artifact. The record looks healthy. The work is not. That is the loop: certify, reject, certify, reject — until someone names the mismatch and changes the method, not the volume of retries.

Cost of a certified failure
